People Incorporated, Team North staff provides services to adults living with chronic mental illness and/or are diagnosed with co-occurring disorders. The goal of our services is to help persons live safely and productively in the community, and to help them develop self-awareness and make positive plans for the future.

Who we serve

We serve adults (18+) living in Hennepin County who have chronic mental illness and/or are diagnosed with co-occurring disorders.

The goal of our services is to help persons live safely and productively in the community, as well as develop self-awareness and make positive plans for the future.

Services we provide

Rule 79, Targeted Case Management Services may include developing a goal plan for each person, helping with housing, daily living, substance use and physical health management, financial management, educational/employment opportunities, and the use of resources and services in the community. All needs and goals are assessed every 6 months to ensure clients needs are being consistently met.

About the process

Service Coordination receives all of its referrals directly from Hennepin County.

About our fees

Services are reimbursed through a combination of Medical Assistance, Managed Health Care Plans and county contracts.

About our staff

All staff hold a bachelor’s degree in a Social Service related field and many have Master’s Degrees. All staff have experience in mental health, chemical dependency, and/or various co-occurring disorders. Regular in-services and trainings are provided for all staff in areas relevant to the needs of the individuals we serve.

Hours of operation

Monday – Friday 8:00 a.m. to 5:00 p.m.; Alternate appointments will be made, as needed, based on individual needs.

Member of the Minnesota Association of Community Mental Health Programs (MACMHP) as well as members of the US Psychiatric Rehabilitation Association (USPRA).
