We engage and support adults without shelter who are experiencing mental illness and homelessness in Hennepin and Ramsey County.
The goals we set
The goal of our street outreach program is to creatively engage individuals experiencing homelessness without a shelter and link them to appropriate health and housing services.
Services we provide
We provide routine street outreach and engagement services to a variety of locations in the community. Services provided during outreach include assistance with survival gear, de-escalation, crisis intervention, and referral to health care, housing, and benefits resources. For individuals who need additional supports, outreach staff provide short term coordination of services in order to successfully link them to housing and mental health supports.
Referrals to the street outreach program come through a variety of community partners, such as case managers, law enforcement, and other social service providers. Self-referrals are also accepted.
