Children’s Mental Health Targeted Case Management works to enable a child to improve or maintain emotional or behavioral functioning and remain with his/her family. The goal is to reduce the need for and reliance upon out-of-home placements and other services that are more intensive, restrictive, and more costly.
Service we provide
Case management services are coordinated services designed to help the child and his/her family obtain appropriate support, including mental and physical health services, social and educational services, vocational and recreational services, and other related support such as volunteer services, advocacy, transportation, and legal services.
Our services include:
- conducting functional assessments
- developing service plans
- assisting clients in obtaining mental health and other services
- ensuring the coordination of services
- monitoring and evaluating the delivery of services
Who we serve
We serve children under the age of 18 who have been diagnosed to have a Severe Emotional Disturbance (SED). The child must be a resident of Hennepin County, or a child for whom Hennepin County has been determined to be the county of financial responsibility.
Clients are referred by Hennepin County, Prepaid Medical Assistance, other People Incorporated programs, or other outside agencies. Either will work with you to determine if your child qualifies for case management services. If approved, services will be paid for by the county or by Prepaid Medical Assistance.
