Our Day Treatment for Children and Youth Grades 2-12 program provides site-based mental health services for children and youth, grades 2-12, who are struggling with functioning at home, in school, or in the community due to a serious mental health diagnosis.
The goal is to stabilize the child’s mental health status, develop and improve the child’s independent living and socialization skills, and enable the child to thrive in their community.
Please email a Day Treatment referral form, along with supporting documentation (Diagnostic Assessment, Psych Evaluation, IEP Evaluation, etc.) to the secure email at email@example.com. If deemed potentially eligible, intakes will be scheduled. If you need further assistance, please call 763-331-3033.
