The Dakota Apartment Program helps waiver-eligible individuals (18 years and older) with a Serious Mental Illness (SMI) diagnosis live more independently in an apartment community setting while still getting the mental health support that they need. Disability waiver-eligible: Community Access for Disability Inclusion (CADI) Waiver, Brain Injury (BI)Waiver, or the Developmental Disabilities (DD) Waiver.
Referrals come from an individual’s county of residence or case manager. Clients must be able to pass a criminal background check. Call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 am – 5:00 pm for openings and eligibility information.
