Corrections Integrated Support
One-third of people incarcerated in Minnesota county jails may be struggling with a mental illness, and many others are undiagnosed. They need help but often there are too few resources available.
Our goal is to collaborate with our corrections system to increase access to mental health and substance use services. Our teams will help identify and connect individuals to available treatment that provides support throughout their recovery process, empowering them to thrive in their communities.
Referrals
Clients are referred through our county correctional partners, and we work to connect them to services that support mental and chemical health, as well as address other social determinants of health. Call Central Access at 651-774-0011, Monday through Friday, from 7:00 a.m. – 5:00 p.m. to learn more.

Clients are referred by our county correctional partners once there is an identified mental health concern.
Our multi-disciplinary team works to rapidly respond to new clients as well as maintaining engagement with existing clients to support overall health and recovery throughout post-transition release:
- Mental health assessments, diagnosis, person-centered mental health, substance use treatment planning, and care coordination
- Post-release transition support is also available, which extends beyond mental health, helping people connect to employment, housing, income programs, vocational/educational counseling, and more
Our team is comprised of Licensed Mental Health Professionals, Mental Health Practitioners, and Peer Support Specialists to meet the unique needs of our community in a person-centered way.
