Supportive Housing with Services
Our Supportive Housing with Services provides a more intensive level of mental health and independent living support in a residential or apartment setting. Our goal is to provide holistic care to help our clients live independently in the community while managing their mental illness, including supporting underserved groups – such as the deaf/hard of hearing communities.
Below are all of our residential and apartment-supportive programs.
For referrals, call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 a.m. – 5:00 p.m.
Deaf Supportive Living
Our Deaf Supported Living program provides culturally competent, integrated mental health services to the Deaf, Deaf-Blind, and Hard-of-Hearing communities, including the primary mode of communication being American Sign Language (ASL).
Eligibility
- 18 years of age or older who are deaf, hard of hearing, or deaf-blind
- Mental health diagnosis
- CADI (Community Access for Disability Inclusion) Waiver or TBI (Traumatic Brain Injury) Waiver eligible
Community Residential Services
- Waiver-eligible adults 18 years of age and older of any gender living in Anoka, Dakota, Hennepin, and Washington Counties in Minnesota
- Serious Mental Illness (SMI) diagnosis
- Long-term housing
