Community Residential Services

This program offers long-term housing and intensive supportive services in a community, residential housing setting. 

  • 18 years of age or older living in Anoka, Dakota, Hennepin, or Washington counties
  • Serious Mental Illness (SMI) Diagnosis
  • Disability waiver-eligible: Community Access for Disability Inclusion (CADI) Waiver, Brain Injury (BI) Waiver, or Developmental Disabilities (DD) Waiver

Our nine residential houses are in Anoka, Dakota, Hennepin, and Washington counties.

Referrals

Call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 am – 5:00 pm for openings and eligibility information.

Two of the nine residences, Penn and Scott House, are for our deaf and hard of hearing or deaf/blind clients.
