Adult Targeted and Waiver Case Management Services
We provide case management services to adults living with serious substance use issues, mental illness, or those with a dual diagnosis of substance use and mental illness. Services are available to individuals living in Anoka, Dakota, Hennepin, Ramsey, and St. Louis counties in Minnesota.
Referrals
Call the Central Access Contact Center at 651-774-0011, Monday through Friday, from 7:00 a.m. – 5:00 p.m. to learn more. Waiver case management referrals must come directly from each county.
Adult Targeted Case Management
Adult mental health targeted case management (AMH-TCM) services help adults with serious mental illness (SMI) gain access to medical, social, educational, vocational, and other necessary services connected to the person’s mental health needs. Targeted case management (TCM) services include developing a functional assessment (FA) and an individual community support plan (ICSP). It also includes referring and linking the person to mental health and other services while ensuring coordination and monitoring of the delivery of services. People Incorporated also provides Children’s Targeted Case Management.
Program Eligibility & Services
- Adults 18 years of age or older who live in Anoka, Dakota, Hennepin, and Ramsey counties in Minnesota
- Diagnosis of a serious mental illness, substance use disorder, or co-occurring disorders
- Services may include developing a support plan for each individual, helping with housing needs, daily living, substance use, mental health and physical health management, financial management, educational/employment opportunities, and the use of resources and services in the community
Adult Waiver Case Management
A service that provides people and their families with access to assessment, person-centered planning, referral, linkage, support plan monitoring, coordination, and advocacy related to waiver services, resources, and informal supports that are not necessarily funded through the waiver.
Program Eligibility & Services
- Adults 18 years of age or older who live in Anoka, Dakota, Hennepin, Ramsey, and St. Louis counties in Minnesota
- Must complete a MNCHOICES assessment and be approved for waiver-funded services or Rule 185 case management through your county of residence.
- Diagnosis of a serious mental illness, substance use disorder, or co-occurring disorders
- Services may include developing a support plan for each individual, helping with housing needs, daily living, substance use, mental health and physical health management, financial management, educational/employment opportunities, and the use of resources and services in the community.
- Individuals served must receive services through the CADI (Community Alternatives for Disabled Individuals) or BI (Brain Injury) waiver. Waiver case managers coordinate services for individuals in any number of the following areas: medical, social, housing, employment, and education.
