Program Overview

Offering coordinated care to adults with multiple chronic health conditions, mental health diagnoses, and substance use disorders. You likely qualify if you are an adult enrolled in Medical Assistance and living with mental illness and are not currently being served by Assertive Community Treatment (ACT) or Targeted Case Management programming. Primarily serving the Twin Cities area.

Program Goals

  • Utilize a person-centered team approach to help clients meet goals and better manage health conditions, including Certified Peer Support Specialists.
  • Offer services to navigate the healthcare system, secure transportation to appointments, maintain housing and employment, get referrals to community supports, and more.

Program Impact

  • Serve over 250 individuals per year with continued growth
  • Assist clients in accessing primary care and specialty clinics to avoid frequent hospital or ER care
