Adult Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is intensive, nonresidential treatment and rehabilitative mental health services provided according to the ACT model – a single, fixed point of responsibility for treatment, rehabilitation and support needs for clients.


Clients may be referred to our programs and services by an insurance provider. Applicants with health coverage through a managed care organization must complete a chemical health assessment through their provider and obtain authorization prior to admission. A diagnostic assessment, discharge summaries from recent hospitalizations and/or previous psychiatric records are helpful to include with your referral.

Referrals can come from anyone even a self-referral as long as a client is Hennepin County of Financial Responsibility (CFR).

  • Individuals must be adults (18+) with a primary diagnosis of schizophrenia, schizoaffective disorder, major depressive disorder with psychotic features or other psychotic disorders or bipolar disorder.
  • They need to be a Hennepin County resident and also we request clients engage with the ACT team 2 or 3 times per week.
