Adult Assertive Community Treatment (ACT)
An intensive, comprehensive outpatient treatment and rehabilitative mental health services program for adults (18+) with a primary diagnosis of schizophrenia, schizoaffective disorder, a major depressive disorder with psychotic features, bipolar disorder, or other psychotic disorders. Individuals need to be Hennepin County residents and engage with the ACT team 2-3 times per week.
Referrals
Clients may be referred to our programs and services by an insurance provider. Applicants with health coverage through a managed care organization must complete a chemical health assessment through their provider and obtain authorization prior to admission. A diagnostic assessment and discharge summaries from recent hospitalizations and/or previous psychiatric records are helpful to include with your referral.
Referrals can come from anyone, even a self-referral, as long as a client is Hennepin County of Financial Responsibility (CFR).
