Heather Ridge is a Washington County, 4-bed, Rule 203 licensed adult foster care home for people with serious and persistent mental illness. We provide a safe environment and teach residents the skills they need to live more independently.
Who we serve
Heather Ridge serves adults (18+), who are identified by Washington County Adult Mental Health as needing a mental health-focused adult foster care. They must be waiver-eligible, including CADI and TBI waivers. People whose crisis is due primarily to substance abuse are not eligible.

The goal of Heather Ridge is to enable each person to utilize and strengthen the skills they have while learning new tools for living independently. The environment for care at Heather Ridge is one that fosters as much independence as possible for the individuals who here.
Services we provide
The program provides a home-like, supportive environment for adults with serious and persistent mental illness to learn independent living skills. We offer assistance with medication set up, medication monitoring, transportation to medical and psychiatric appointments, independent living skills training, socialization, crisis intervention/ prevention, meal planning, cooking, one-on-one interaction, appointment scheduling, and daily check in with staff. Each individual’s routine or daily schedule is determined by his/her interests and needs as defined by the person and the care team. The program has four beds for long-term participants, and provides 24-hour supervision.

Individuals are referred to Heather Ridge through Washington County Adult Mental Health.
About our fees
Cost for services at Heather Ridge is covered by individual income, GRH funding, and waiver funds.
About our staff
Staffing at Heather Ridge includes personnel with a minimum of a high school diploma or GED and BA/BS or MA/MS degrees in social work or related fields. Ongoing training at Heather Ridge includes, but is not limited to, CPR, First Aid, OSHA, HIPAA, requirements for working with Vulnerable Adults, Principles and Practices of Psychiatric Rehabilitation, and psychiatric medication training.
Hours of operation
Heather Ridge is staffed 24 hours per day, 7-days a week, with awake overnight staff.

Heather Ridge is a licensed Rule 203 Adult Foster Care program.

Member of the Minnesota Association of Community Mental Health Programs (MACMHP) as well as members of the US Psychiatric Rehabilitation Association (USPRA).
