Adult Foster Care – Summit Park

People Incorporated Summit Park Supportive Housing staff provides services to adults with a variety of mental health diagnoses or disabilities.

Who we serve
There is no requirement that clients (adults 18+) served by Summit Park have a particular diagnosis. Most clients have a significant mental health component in their service plan. Their diagnoses or disabilities may include mental illness, traumatic brain injury, learning disabilities or cognitive impairment, and other related conditions.

The goals we set
The goal of our services is to help our clients live safely and productively in the community, helping them develop self- awareness and make positive plans for their future.

Services we provide
Summit Park is a step between a traditional foster care house and independent living. The residents reside in two bedroom apartments and have access to staff if they are in need of support. Staff checks on residents throughout the day to ensure their safety and to help solve problems that arise. Staff encourages residents to solve problems on their own, strengthening their independence.

About the process
Referrals are received through Dakota County workers. Referrals from other counties will be considered depending on occupancy.

About our fees
CADI, DD or TBI waiver required to pay for foster care.

About our staff
Services are provided by Mental Health Rehab Workers and Mental Health Practitioners who have extensive experience working in the social service field.

Hours of operation
Staff members are available at all times, either on site or reachable by staff cell phone.
