Adult Foster Care in Dakota County (Colonial House, Foliage House, McAndrews House, River Hills House) is a 4-bed, Rule 203 licensed program for people with serious and persistent mental illness. We provide a safe environment that helps individuals become more self-sufficient.
Who we serve
Colonial House, Foliage House, McAndrews House, and River Hills House serve adults (18+) who are identified by Dakota County Mental Health as a candidate for Adult Foster Care. All potential residents must be CADI/TBI waiver eligible.
The goals we set
The goal of each home is to help individuals uncover their inner strengths and to move towards as much independence as possible.
Services we provide
The program provides a home atmosphere for adults with serious and persistent mental illness to learn independent living skills. We offer medication management, transportation to appointments, independent living skills training, education groups, socialization, crisis intervention/prevention, meal planning, cooking, one-to-one counseling, appointment scheduling, and daily check in with staff. Each individual’s routine or daily schedule is determined by his/her interests and needs as defined by the person and the care team.
About our location
All four homes are located in the heart of residential neighborhoods in Dakota County. They are all within close proximity to parks, biking/walking trails, bus lines, business, shopping, and restaurants. The environment fosters as much independence as possible. The program has four beds for long-term participants and provides 24-hour (including awake overnight staff) supervision.
About the process
Individuals are referred by Dakota County Adult Mental Health. After a referral from Dakota County Mental Health, an intake session is scheduled with prospective clients to ensure compatibility between the program and the client’s needs. The intake includes a detailed description of our program and expectations, as well as a tour of the house.
About our fees
Room and board are paid for by a combination of GRH funds and client payments. Services are paid for by CADI/TBI funds.
About our staff
Staffing at all the homes include personnel with a minimum of a high school diploma or GED and BA/BS or MA/MS degrees in social work or related fields. Ongoing training for all employees includes, but is not limited to CPR, First Aid, OSHA, HIPPA, VA, and medication administration.
Hours of operation
The program is staffed 24 hours a day, 7 days a week, with awake overnight staff.
