Project SAIL (Support and Advocacy for Independent Living)

Who we serve
We assist chronically homeless adults with a mental illness to achieve their goals related to safety, stability and recovery. For chronically homeless individuals living on the street, the transition from the street to permanent housing is made in stages (as necessary) and staff work to create a service environment where individuals feel at ease, out of danger, with flexible program services to meet their needs.

The goals we set
Through mobile support we work to develop relationships with the people we serve. We assist them with housing needs, physical and mental health care, and independent living skills.

Services we provide
We provide community outreach and housing support. We offer medication and assistance, education, skills development, crisis assistance, resource development and coordination, case management, and medical and psychiatric coordination.

About our location
Project SAIL provides services in Ramsey, Washington, Anoka, and Dakota counties.

About the process
Individuals are referred to the mobile support team through community outreach case managers.

About our fees
Cost for the mobile support team is covered through Hearth Connection, a Long-Term Homeless Supportive Services Fund.

About our staff
A registered nurse, and bachelor level mental health practitioners/case managers. We also have a contracted psychiatrist on staff.
