Who we serve
We assist chronically homeless adults with a mental illness to achieve their goals related to safety, stability and recovery. For chronically homeless individuals living on the street, and in shelters the transition to permanent housing is made in stages.

The goals we set
The goals of this street outreach program are to increase the level of safety, stability, and recovery among chronically homeless individuals with serious and persistent mental illness in Ramsey county.

Services we provide
This service provides street outreach, basic survival gear, assistance in stabilizing mental illness symptoms, benefit assistance, and housing placement for Hennepin & Ramsey county homeless, unsheltered adults with mental illness who may or may not have chemical dependency.

About our location
We provide outreach to persons living in homeless camps and in shelters.

About the process
Referrals to the street outreach program come through a variety of community partners, i.e. case managers, law enforcement, and other social service providers.

About our fees
Cost is covered by a variety of grant resources as well as a HUD services-only grant.

About our staff
Bachelor level street outreach workers, a registered nurse, and a consulting psychiatrist.

Hours of operation
Various hours as needed.
