We assist chronically homeless adults with a mental illness to achieve their goals related to safety, stability and recovery. For chronically homeless individuals living on the street, the transition from the street to permanent housing is made in stages (as necessary) and staff work to create a service environment where individuals feel at ease, out of danger, with flexible program services to meet their needs.
The goals we set
Through mobile support we work to develop relationships with those we serve. We assist them with housing needs, physical and mental health care, and independent living skills.
Services we provide
We provide community outreach and housing support. We offer medication and assistance, education, skills development, crisis assistance, resource development and coordination, case management, and medical and psychiatric coordination.
About our location
We provide supportive services to a group of formerly homeless clients in their apartments while also using the Wycliff Office as a base for our meetings and community involvement with the clients.
About the process
Referrals to the service come through The People Incorporated Outreach teams and the People Incorporated Transitional Houses.
About our fees
Various grants cover cost.
