Northside Community Support Program

The Northside Community Support Program (NCSP) is a resource center for adults with a mental illness. We help people recover from mental illness by helping them learn skills and developing supportive services. At our center members work on a variety of individual goals at their own pace. People develop long-term relationships with staff and other members, and through this support, members renew their sense of dignity and hope. The program can serve as an access point for people to help them get connected to other services.

Integrated Service Network: We are part of a larger Integrated Services Network that includes Outpatient Chemical Dependency Treatment, Intensive Mental Health Residential Treatment and Adult Foster Care Program. All four programs are located on a single campus.

Who we serve
We welcome any adult resident (18+) of Hennepin County who has a serious mental illness. Many members also experience substance abuse or have other co-existing disorders. We prioritize services to people who are in crisis or at risk of losing community tenure. We frequently assist with immediate needs for persons at risk, while we are waiting to determine if a person will ultimately be eligible.

The goals we set
We measure our success by the number of people who successfully locate and maintain housing, the access we provide to those who need community support, quality of life, and client satisfaction. We also track the number of people served and the types of services provided. Our services are designed to promote stability and recovery.

Services we provide
The Northside Community Support Services include: housing assistance, benefits assistance, supportive counseling, peer support, recreational activities, psychosocial educational and support groups, community outreach, community education, family education, MI/CD support groups, and referral to other community supports to help people manage their illness and recovery. We also provide Targeted Case Management Services for up to 35 clients. We have a Mental Health Initiative Team on-site that provides Target Team Case Management to approximately 100 clients.

About our location
Our center is located in north Minneapolis and is very accessible by bus and near many conveniences. Based on need, services are provided on site, at community locations or in the client’s home.

About the process
No referral is needed for NSCP. People can walk in to start the process. We do need verification of a serious and persistent mental illness, but we often help people with immediate needs while we wait for the verification.

About our fees
CSP services are provided through a contract with Hennepin County and there is no cost to the client. Case management services are billed to Medical Assistance or to the clients Managed Care Organization if the plan covers these services.

About our staff
Our staff has a high degree of expertise in mental health, chemical dependency, and community resources. The majority of the staff holds a BA or Masters degrees, and all participate in continuing education.

Hours of operation
The program is open: 8:30 a.m. – 5:00 p.m. on Monday, Tuesday, Thursday, and Friday 8:30 a.m. – 7:00 p.m. on Wednesday
