Service Coordination in Ramsey County provides case management services for adults with a mental illness.
Who we serve
Adults and children with a mental illness and who receive services through the CADI (Community Alternatives for Disabled Individuals) or TBI (Traumatic Brain Injury) waivers.
The goals we set
Goals are set in the form of a Community Support Plan, and are individualized to meet the person’s needs and offer support across all environments.
Services we provide
Service Coordinators coordinate services for individuals in any number of the following areas: medical, social, housing, employment, and education. The coordination of services includes the management of waiver money allocations and the reporting of services to the County as needed. Service Coordinators work directly with the individual and members of their team to create a supportive environment while encouraging independence.

Service Coordination receives all of its referrals directly from Ramsey County.
About our fees
Service coordination services are paid for through the individual’s waiver. Some payment may be required based on the number of services used and the type of funding the individual has.
About our staff
All staff are Licensed Social Workers (LSW), and many have Master’s Degrees. All staff have experience in mental health, chemical dependency, and/or developmental disabilities. Regular in-services and trainings are provided for all staff in areas relevant to the needs of the individuals we serve.
Hours of operation
Monday – Friday 8:00 to 5:00; Alternate appointments will be made, as needed, based on individual needs.
Affiliations
Member of the Minnesota Association of Community Mental Health Programs (MACMHP) as well as members of the US Psychiatric Rehabilitation Association (USPRA).
