Mental Illness/Chemical Dependency Case Management – St. Joseph’s Hospital

Service Coordination and Case Management is a collaborative effort between HealthEast Care System, People Incorporated and Ramsey County. We strive to provide community based services using a holistic approach that includes support, advocacy and life management skills that will assist clients with their recovery needs and promotes overall health.

Who we serve
An eligible recipient is a Minnesota resident who is eighteen years or older, chemically dependent with active Medical Assistance, MN Care or private insurance, diagnosed as chemically dependent or dually diagnosed with a mental illness and referred by a participating county chemical health department and eligible for Rule 25 funding.

The goals we set
We strive to provide community based services using a holistic approach that includes support, advocacy and life management skills that will assist clients with their recovery needs and promotes overall health.

Services we provide
Staff meet with clients to provide services including, but not limited to: assessment of chemical dependency and abuse, including Rule 25 assessments; individual counseling with emphasis on chemical use and mental health based on client’s stage of change; symptom management, relapse prevention and crisis intervention; medication management referrals and education; treatment transition to community living from sober, transitional and permanent housing referrals; and group counseling.

About our fees
Services are funded by County Consolidated Treatment Funds, MN Care, Medical Assistance and PMAP (Prepaid Medical Assistance Plan) or Private Health Insurance (contingent on coverage).

About our staff
Services are provided by Licensed Alcohol and Drug Counselors who are trained and experienced in providing treatment to adults with mental illness and chemical dependency.

Hours of operation
8 a.m. to 4:30 p.m., Monday through Friday

Licensing
The program meets contract requirements of the county and state-mandated requirements for Rule 31 Service Coordination.

Member of the US Psychiatric Rehabilitation Association (USPRA) and MN Association of Community Mental Health Programs (MACMHP).
