Transitions
Our Transitions program is for those with intractable epilepsy and helps each participant prepare for successful independent living. It offers round-the-clock support and several hours of independent living skills training per week. Participants—who can come from anywhere in Minnesota—live in their own private apartment in The Cedars apartment complex or out in the community? Most participants stay in the program for two years or longer, gaining the skills and confidence they need to live more independently.
Hennepin County Assisted Living
The Hennepin County Assisted Living program provides long-term support for adults in Hennepin County who need ongoing, 24/7 support to maintain stability. Participants also live in their own private apartments at The Cedars. Services are tailored to the needs of each individual, and can include housekeeping, meal preparation, and laundry services.

About Living at The Cedars
The Cedars is located near downtown Minneapolis and the West Bank of the University of Minnesota in an area known for its theaters and restaurants. It is a multi-cultural hub that is served by six public bus routes and the Minnehaha Corridor Light Rail line, so there is quick access to stores nearby.
When you live at The Cedars you get your own private, one-bedroom apartment, including your own bathroom and kitchen. The subsidized apartments are newly renovated and include new air conditioning. Pets are allowed and there are gardens on the premises. Management is on site 40 hours a week.
If you decide to live at The Cedars, you retain your lease agreement with them, even if you complete the Transitions program. The Cedars gives you a safe and affordable home now and in the future. |
